VA seeks audiology services
The Department of Veterans Affairs, VA Roseburg Healthcare System, Roseburg, OR, requires a contractor to provide audiology services which will include the following:
- 1) audiogram;
2) hearing aid evaluation/fitting;
3) binaural hearing aid fitting;
4) monaural hearing aid fitting; and
5) earmold.
The contractor shall perform services at contractors site, which shall include all labor and materials to perform the audiology services required in the contract. The VA may elect to award single or multiple contracts under this solicitation.
Contractors must have offices within the county they are bidding on. The following counties must be served: Coos, Curry, Douglas, Lane, Jackson, Josephine, and Klamath. This requirement is a base year, plus four one-year options contract pending availability of funds. Contract performance will be required from May 1, 2001 through April 30, 2002 as the base year. This solicitation is set-aside for 100% small business. NAICS Code 621340/$5 million applies.
